30-Day Post-Caesarean Recovery Protocol
This clinical protocol outlines the 30-day home recovery pathway for a post-Caesarean section patient.
To ensure the safety of the patient and evaluate the quality of home nursing execution, the clinical team monitors five core KPIs daily:
- Wound Re-Epithelialization Rate: 0% incidence of wound dehiscence or secondary infection
- Pain Optimization Metric: VAS pain score ≤3/10 during rest
- Mobility & VTE Risk Mitigation: Independent ambulation 3x daily by Day 7
- Lactation Re-Supply: Successful infant latch matching growth requirements
- Early Psychological Screening: Weekly EPDS evaluation
Focus: Incision protection, hemodynamic stabilization, pain control, and early mobilization.
Days 1-3: Immediate Stabilization
Evaluate vital signs every 4 hours. Inspect dressing for bleeding. Assess fundal height and lochia. Administer scheduled pain medications. Support incision with pillow during movement.
Days 4-5: Dressings & Digestive Function
Assess bowel sounds and function. Remove surgical dressing if approved. Clean wound with sterile saline. Provide lactation support for breast engorgement.
Days 6-7: Suture Verification
Screen for DVT signs. Monitor for systemic infections. Coordinate suture removal. Transition to independent mobility around the house.
Focus: Safe movement, managing internal swelling, and stabilizing maternal routines.
- Day 8: Detailed incision assessment. Transition to on-demand oral pain management.
- Day 9: Evaluate iron status and screen for anemia. Review diet for fiber, protein, and fluids.
- Day 10: Assess lochia transition from rubra to serosa. Ensure uninterrupted rest periods.
- Day 11: Introduce gentle pelvic floor exercises (Kegels) and diaphragmatic breathing.
Phase 4: Independence & discharge (days 22-30)
Focus: Full functional independence and preparation for discharge from home care.
- Final wound assessment and clearance
- Complete psychological screening
- Discharge planning and follow-up scheduling
- Education on warning signs requiring medical attention
